NO INSURANCE ACCEPTED. For submittal, receipts can be provided.
APPOINTMENTS MUST BE CANCELLED AT LEAST TWENTY-FOUR (24) HOURS IN ADVANCE.
New and follow-up patients who miss their scheduled appointment time or do not cancel twenty-four (24) hours before the appointment will be charged $250.00 for New Patient Appointments and $150.00 for Follow-up Appointments. This is strictly enforced. Canceling on a weekend day or holiday is not considered twenty-four-hour notice.
MSICS MENSSANA, P.C. Provider has to see a patient for a minimum of 6 visits before a determination can be made for FMLA paperwork. MSICS MENSSANA does not specialize in FMLA cases. There is a fee for form completion. For disability paperwork, the patient has to be seen six visits before, and a determination can be made for any disability paperwork. We do not specialize in disability cases/paperwork. A fee will be charged for the paperwork.
There is a $25.00 basic fee for documents. Additional fees may pertain to additional pages.
